Short answer: the most telling pattern is a non-contact twist, cut, or landing with a pop at the moment of injury, swelling that fills the knee within a few hours, trouble fully straightening it, and a sense that the knee might buckle. A clinical exam checks ligament stability directly, and an MRI confirms the tear and any meniscus or cartilage involvement when that detail will change the plan.

The Signs Clinicians Look For
- A pop or snap felt at the moment of injury
- Swelling within two to six hours rather than days later
- Loss of full knee extension
- A feeling of shifting, buckling, or not trusting the leg
- A non-contact mechanism: plant and cut, hard stop, or awkward landing
Grades 1 Through 3
Grade 1 means the ligament was stretched but the knee still checks out stable. Grade 2 is a partial tear with some added laxity. Grade 3 is a complete tear where the ligament no longer restrains forward and rotational movement, and stability testing is clearly positive. Grade does not fully determine treatment, because your goals and how the knee behaves under load carry real weight.
What an Exam Includes
A thorough assessment reviews the mechanism, checks swelling and range of motion, tests ligament stability, screens the meniscus, and assesses quad activation. It should also look at the hip and ankle, since those joints absorb load the knee cannot and often stiffen quickly after an injury.
When Imaging Is Worth It
Imaging is warranted when surgery is on the table, when the knee locks or catches and a meniscus tear is suspected, when swelling and instability persist, or when the exam findings do not match the story. X-rays rule out fracture; MRI shows the ligament and soft tissue.
Go To Urgent Care Instead If
The knee cannot bear any weight, will not straighten at all, looks deformed, or the foot feels numb or cold. Those need urgent medical evaluation first.
