Runner's Knee: Why It Hurts and How to Run Again Without Pain

Short answer: runner's knee is usually a load problem at the kneecap, not damage inside the joint. It improves when hip and calf strength go up, cadence and mileage progression are adjusted, and restricted hips or ankles are freed up. Most runners feel better in three to six weeks and keep running in a modified form the whole time.

What It Feels Like

Aching around or under the kneecap that shows up on downhills, stairs, squatting, or after sitting for a long stretch. It often starts vague and mild, then becomes predictable at a certain distance or grade. Arvada and foothills running, with its long descents, tends to bring it out fast.

Rarely all the way. Most runners can keep flat, easy mileage under a mild pain threshold with no next-day flare, while descents and speed work pause for a few weeks.

Pain-free stairs and squatting, single-leg control without the knee collapsing inward, and a full easy run with no next-day soreness before adding hills or intensity.
